INVITATION TO TENDER FOR COVID-19 INTERVENTION PROJECTS
(EMERGENCY PROCUREMENT)
1. INTRODUCTION
The University of Uyo Teaching Hospital (UUTH), Uyo, Akwa Ibom State, a tertiary health institution in implementing her 2020 Appropriation for COVID-19 Intervention project and in compliance with Public Procurement Act 2007, hereby invites interested, qualified and reputable contractors with relevant experience and track record to tender for the following projects:
2. PROJECT DESCRIPTION
LOT 1. Procurement & Installation of Molecular Laboratory Equipment
LOT 2. Equipping of 10 bedded Intensive care unit (ICU)
LOT 3. Equipping of Isolation center
LOT 4 Procurement of Personal Protective Equipment
3. ELIGIBILITY REQUIREMENTS
- Evidence of Certificate of Incorporation issued by Corporate Affairs Commission (CAC) including Form CAC 1.1 or CAC2 and CAC7;
- Evidence of Company’s Income Tax Clearance Certificate for the last three (3) years valid till 31st December, 2020;
- Evidence of current Pension Compliance Certificate valid till 31st December, 2020;
- Evidence of current Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2020;
- Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31st December, 2020;
- Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 31/12/2020 or valid Certificate issued by BPP.
- Sworn Affidavit disclosing whether or not any officer of relevant committees of the University of Uyo Teaching Hospital or the Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars;
- Company’s Audited Accounts for the last three (3) years -2017, 2018, 2019;
- Evidence of financial capability to execute the project by submission of Reference Letter from a reputable commercial bank in Nigeria indicating willingness to provide credit facility for the execution of the project when needed;
- Company Profile with the Curriculum Vitae of Key Staff to be deployed for the project, including copies of their Academic/Professional qualifications.
- Verifiable documentary evidence of at least three (3) similar jobs executed in the last five (5) years including Letters of award, Valuation Certificates, Completion Certificates and Photographs of the projects. Only Supplier with exceptional and convincing exposure and experience in Molecular Laboratory Equipment and ICU Equipment will be considered for LOT 1 and LOT 2 respectively.
- Letter of Authorization from the Original Equipment Manufacturers (OEM) listed in the Approved Policy for Procurement of Health and Medical Equipment for Tertiary Hospital in Nigeria, for bidders of Lots 1, 2 &3;
- For Joint Venture/Partnership, Memorandum of Understanding (MoU) should be provided (CAC, Tax Clearance Certificate, Pension Compliance Certificate, ITF Compliance Certificate, NSITF Compliance Certificate, IRR & Sworn Affidavit are compulsory for each JV partner)
- All documents for submission must be transmitted with a Covering/Forwarding letter under the Company/Firm’s Letter Head Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM No.), and e-mail address. The Letterhead Paper must bear the Names and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorized officer of the firm.
4. COLLECTION OF TENDER DOCUMENTS
Interested Companies are to collect the Standard Bidding Document (SBD) from the Procurement Unit, University of Uyo Teaching Hospital on evidence of payment of a nonrefundable tender fee of N10,000.00 per Lot, paid into the University of Uyo Teaching Hospital‘s Remita account in any commercial bank in Nigeria.
5. SUBMISSION OF TENDER DOCUMENTS
Prospective bidders are to submit bid for each of the Lot desired, two (2) hard copies each of the technical and financial bids with softcopy of financial bid only in MS Excel format, packaged separately in sealed envelopes and clearly marked as “Technical Bid” or “Financial Bid”. Thereafter, put the two sealed envelopes together in a bigger sealed envelope addressed to The Chief Medical Director, University of Uyo Teaching Hospital, Uyo, P.M.B 1136, Uyo, Akwa Ibom State and clearly marked with (the name of the Project and the Lot number). Furthermore, the reverse of each sealed envelope should bear the name and address of the bidder and drop in the designated Tender Box in the Conference Room 2nd Floor, Administration Block not later than 12:00 noon on Monday, December 14, 2020. Bidders should sign the Bid Submission Register at the point of submission.
6. OPENING OF TECHNICAL BIDS
Only the technical bids will be opened immediately after the deadline for submission at 12:00 noon on Monday, December 14, 2020 in the Conference Room 2nd Floor, Administration Block, UUTH.
7. GENERAL INFORMATION
- All documents may be referred to relevant Government Agencies;
- Bids must be in English Language and signed by an officer authorized by the bidder;
- Bids submitted after the deadline for submission would be returned un-opened;
- Bidders should not bid for more than two (2) Lots;
- All costs will be borne by the bidders;
- Bidders are excused from the bid opening, which will be attended by officials of private sector professional organization and CSOs in the areas of anti-corruption/transparency. The event will be fully captured on video and can be provided to bidders on request;
- Standard Bidding Documents can also be collected in Soft copies and Bidders can submit through Post (Courier Services); and
- Only pre-qualified bidders at technical evaluation will be invited at a later date for financial bids opening, while the financial bids of un-successful bidders will be returned un-opened.
8. DISCLAIMER: The University Of Uyo Teaching Hospital is not bound to pre-qualify any bidder and reserves the right to annul the Procurement process at any time without incurring any liabilities in accordance with Section 28 of the Public Procurement Act 2007.
